Special Diet Information
Nutrition Services cannot make accommodations for your student unless a Special Dietary Form is on file. This form does not need to be updated annually and is valid until we receive either an updated Special Dietary Form or a Discontinuation Form.
**Nutrition Services must be notified and the form filled out even if the school nurse has record of the allergy.**
